We are happy that you want to take the next step with AchievePoint Academy Arizona! If you plan to use an ESA to cover your tuition, the enrollment process does require that you meet specific qualifications under the Arizona Department of Education Empowerment Scholarship Account (ESA) Program.
We do not administer the Empowerment Scholarship Application process, but we’re here to help you through it. Below find essential information from the Arizona Department of Education’s website regarding eligibliity for ESAs and how to apply.
To qualify for ESA, you must meet the eligibility criteria for one of the following categories:
• Currently Attending a D or F Rated School
Eligibility
1. The Student and Applicant must be residents of Arizona
2. The Applicant (the person applying) must be the legal parent or guardian
3. If the student is eligible to attend 9th-12th grade for the application year: they must be attending the D/F school at the time of application. That means if you are applying on 8/1, the Arizona Department of Education needs to see enrollment in a D/F school overlapping 8/1.
4. If the student is eligible to attend 9th-12th grade, the student must meet the first 100 day requirement. This means the student must have attended the first 100 days of the school year prior to the application year with no more than 10 absences between the first day of school and the 100th day of school (usually mid-March).
- To verify the 100th day, check the public school’s website to locate their calendar to know what was the 100th day. Please also call the school to confirm attendance.
- Students are allowed to miss 10 days or less during the first 100 days of school.
- This requirement can also be met with online hours:
- 9th-12th: 500 Hours or 30,000 Minutes
How can I know if my student is attending a D/F rated school?
Visit the Arizona State Board of Education’s website at: https://azsbe.az.gov/f-school-letter-grades. There is a link that has 2018-2019 A-F Letter Grades on this page which will open an Excel file.
You can search for your school to determine their letter grade. If letter grade is D or F you can apply for ESA under this eligibility.

To provide proof of residency you must submit ONE of the following:
- Real Estate Deed or Mortgage Documents
- Property Tax Bill
- Residential Lease or Rental Agreement
- Water, Electric, Gas, Cable, or Phone Bill
- W-2 Statement

• Dependent of Active Duty Military Personnel
Eligibility
1. The Student and Applicant must be residents of Arizona
2. This eligibility type does not need to meet the first 100 day requirement.
3. The Applicant (the person applying) must be the legal parent or guardian and:
- The Applicant is an active duty (not reserve) service member, in either the Marines, Army, Air Force, Navy, or Coast Guard.
-OR-
- The Applicant is a dependent of the military service member (who is also the legal parent or guardian of the Student) in either the Marines, Army, Air Force, Navy, or Coast Guard.

Documents Required:
1. Student’s Birth Certificate
- The applicant (the person applying) must be listed on the birth certificate (or other legal documentation).
- If the applicant’s legal name at the time of application is not on the birth certificate, documentation of legal name change must be provided to show guardianship.
2. Proof of Residency Document in the Applicant’s Name (the Parent Applying) (Please provide only one of the following documents listing the PHYSICAL address — no PO Boxes.) Under the military eligibility, if you are unable to provide this document because you have not yet moved to Arizona, please contact ESA (602-364-1969 or esa@azed.gov) for more information.
- Valid Arizona driver’s license, Arizona identification card
- Valid Arizona motor vehicle registration
- Valid Arizona Address Confidentiality Program authorization card
- Property deed
- Mortgage documents
- Property tax bill
- Rental agreement or lease (including Section 8 agreement or off-base military housing)
- Utility bill (water, electric, gas, cable, phone)
- Bank or credit card statement
- W-2 wage statement
- Payroll stub
- Certificate of tribal enrollment (506 Form) or other identification issued by a recognized Indian tribe located in Arizona
- Other documentation from a state, tribal, or federal agency (Social Security Administration, Veterans’ Administration, Arizona Department of Economic Security, etc.)
- Temporary on-base billeting facility (for military families)
- A notarized Affidavit of Shared Residence if you don’t have a proof of residence in your name.
3. Current Orders (issued within 60 days)
4. Commanding Officer Letter (issued within 60 days)
- Must state all of the following:
- Name of service member
- Student (and spouse, if applicable) is listed as Dependent
- Location of duty station
- Current status (must be Active)
- Branch

• Residing on a Native American Reservation
Eligibility
1. The Student and Applicant must be residents of Arizona and reside within a Native American Reservation.
2. The Applicant (the person applying) must be the legal parent or guardian.
3. If the student is eligible to attend 9th-12th grade, the student must meet the first 100 day requirement.
- This means the student must have attended the first 100 days of the school year prior to the application year.
- To verify the 100th day, check the public school’s website to locate their calendar.
